NYC Comptroller's Office Substitute Form W-9

In conjunction with the Mayor’s Office of Contracts, the Bureau of Accountancy (Division of Accounting Systems) is responsible for validation of new vendors and maintenance of payee/vendor information in the central database that is part of the City’s Financial Management System (FMS) through the collection of Substitute Form W-9. The following Substitute Form W-9s have been modified to meet the informational needs of FMS and should be used in accordance to agency request.

  [For use by payee/vendors that supply services to City Agencies]
  
• Condemnation Proceeding Substitute Form W-9
  Instructions for Condemnation Proceeding Substitute Form W-9
  [For use by payees that have a condemnation proceeding with the City of New York]
