Procurement Process

The City of New York's procurement process is governed by the Rules of the Procurement Policy Board (PPB rules).   Vendors who wish to do business with the City, may be required to complete the following questionnaires and forms:  

#1 - City of New York - VENDEX Questionnaire and Guides
            - Mayors Office of Contracts

#2 - Tax Affirmation - Department of Finance

#3 - Supply & Service Employment Report - Division of Labor Services
     - Less than 50 Employee Certificate

#4-  Substitute Form W-9 Request for Taxpayer Identification Number & Certification 
           (Instructions- for the Substitute Form W-9)

#5-  Doing Business Data Form - Pension Related

#6-  Doing Business Data Form - Standard Form
