2. Tax Affirmation – Department of Finance
3. Supply & Service Employment Report – Division of Labor Services
4. Substitute Form W-9 Request for Taxpayer Identification Number & Certification
5. Doing Business Data Form – Standard Form – Mayor’s Office of Contract Services
