Community Healthcare Network (CHN) offers comprehensive primary care for Persons of trans experience, HIV prevention including PrEP/PEP, STI screening and treatment (including HIV), groups, treatment adherence, behavioral health, and support services. CHN has clinics in the Bronx, Brooklyn, Manhattan, and Queens as well as Mobile Health Van.
